The first 30 days after a hospital discharge carry a disproportionate risk of readmission. Infection, medication errors, poor nutrition and falls are the most common causes — and most are preventable with the right support in place.
Our Hospital-to-Home programme was designed specifically to bridge this gap. Working in coordination with hospital discharge teams, we ensure that care begins on the day of discharge, with a carer who has been briefed on the client's specific needs and recovery plan.
Our clinical governance is led by a Group Clinical Director for Infection Prevention and Control at an NHS Foundation Trust, with over 20 years of experience. The infection control and monitoring protocols we apply at home reflect current NHS practice.
